Thank you for booking into Pathways to Hospitality and Tourism.

Your booking covers both training sessions. Session 2 will be held in the week following Session 1.

To help us prepare for the training and make sure the session is safe, accessible and relevant, please complete this Participant Information Form before attending.

This form asks for additional details about the participant’s goals, interests, support needs and any adjustments that may help them take part in the training.

It also includes important information about what to expect, what to bring, arrival details, training requirements and next steps.

The information you provide will only be used to support training preparation, communication and next steps after the program.

Before You Complete the Form

Please have the following information available if applicable:

  • Emergency contact details
  • Provider or support coordinator contact details
  • Accessibility or support requirements
  • Relevant medical or health information
  • Dietary requirements for light snacks
  • Any questions you would like to discuss before training

Before You Attend

After submitting your Participant Information Form:

  • The HDNWA team will review your information.
  • You will receive any important updates before your training.
  • If we need any additional information, we will contact you.
  • If your circumstances change before the training, please let us know as soon as possible.

Please note: meals are not provided, so participants may wish to bring their own food or drinks if required.
